Immunization, First Aid and Police Records

Medical Student Requirements

Year 1-4

Based on Schulich Medicine's Immunization Policy and Blood Borne Pathogen Policy, as well as policies developed by the Council of Ontario Faculties of Medicine (COFM), Medical students must provide the following documents:

Year 1-4

Year One (Class of 2029)

Deadline to complete health clearance - August 1, 2025

New this year: All documentation with a potential to expire must stay valid until April 1 2026.  Documentation that will expire before then will be treated as expired and you will fail the Synergy ERV appointment.  Please make sure your documentation will remain current until at least this date.

London and Windsor Students

Year Two (Class of 2028)

Deadline to complete health clearance - August 1, 2025

New this year: All documentation with a potential to expire must stay valid until April 1 2026.  Documentation that will expire before then will be treated as expired and you will fail the Synergy ERV appointment.  Please make sure your documentation will remain current until at least this date.

MD/PhD Students

Deadline to complete health clearance - December 1, 2025

These requirements apply to MD/PhD students during the graduate studies (PhD) portion of their training.

New this year: All documentation with a potential to expire must stay valid until April 1 2026.  Documentation that will expire before then will be treated as expired and you will fail the Synergy ERV appointment.  Please make sure your documentation will remain current until at least this date.

  • Proof of Influenza vaccination or Flu Shot Waiver Form must be uploaded to Synergy Verified by December 1st.  Please follow the unique instructions for this process | Instructions
  • Proof of COVID vaccination or COVID vaccination Waiver Form must be uploaded to Synergy Verified by December 1st. 
  • Criminal Record Disclosure Form 
  • Updates to expired Immunizations as required must be uploaded to Verified
  • Updates to CPR/First Aid -compliance with this requirement is monitored by the School and is NOT part of your Verified Clearance

Year Three (Class of 2028)/ASPIDG 2
