Palliative Care Grand Rounds - Driving Change: The Road to Providing High-Quality Palliative Care Within the West Region of Ontario Health

Join us for Palliative Care Grand Rounds on Wednesday, October 11, to be held via ZOOM from 8:30 to 9:30 a.m.

Lora VanBerlo, BScN, MSc, NP-Adult, Dr. Jitin Sondhi, HBSc, MD, FCFP(PC), and Laura Sheridan RN(EC), MScN, will present, "Driving Change: The Road to Providing High-Quality Palliative Care Within the West Region of Ontario Health."

Session Learning Objectives:

  1. Understand the role of the West Region Palliative Care Network in relation to the provision of high-quality palliative care across the west region.
  2. Utilize data and key performance indicators to identify barriers, trends, and opportunities for
    needed systems level change to improve patient level outcomes.
  3. Develop an awareness of the Ontario Palliative Care Network’s key guidance documents along
    with Health Quality Ontario’s Palliative Care Standard.
  4. Understand funded palliative care educational opportunities offered by Ontario Health West
    Region.

Overall Learning Objectives for 2023-2024 Palliative Care Grand Rounds:

By the end of this program, participants w ill be able to:

  • Identify barriers and trends in palliative care and their impact on patient care.
  • Apply pertinent and relevant palliative medicine know ledge and guidelines to their practice.
  • Discuss the challenges of providing palliative care w ith other primary care physicians, medical trainees, and allied health professionals throughout Southw estern Ontario.

This one-credit-per-hour Group Learning program meets the certification criteria of the College of Family Physicians of Canada and has been certified by Continuing Professional Dev elopment, Schulich School of
Medicine & Dentistry, Western Univ ersity for up to 10.0 Mainpro+ credits.

The 2023-2024Palliative Care Grand Rounds is a self-approv ed group learning activ ity (Section 1) as defined by the Maintenance of Certification Program of the Royal College of Physicians and Surgeons of Canada.

Each participant should claim only those hours of credit that they actually spent participating in the educational program.
