This is a (1) block rotation that provides exposure to primarily adult and occasionally pediatric and geriatric emergency psychiatric assessments and management. These cases will deal with a comprehensive array of medical-psycho-social crisis including, but not limited to; chronic and acute psychosis, severe manic or depressive episodes, and coping crisis that may or may not include suicidal and/or homicidal presentations. There will be a focus on making proper diagnosis, emergency management of the agitated patient, basic psychopharmacology, and assessment of patients in order to properly fill out a Form 1 and a Form 3. This rotation may involve working with a medical student and/or a senior psychiatry resident doing a senior level CEPS elective. The current model also has consistency of working with one of two psychiatric consultants during the course of the week. This is a M-F rotation from 8:30am to 5pm, not including on call expectations.

This rotation may also include some time in the urgent psychiatric clinic for off service residents in order to give a more broad exposure of crisis management. This additional experience is depending on availability of supervision at the urgent clinic and will be determined during the course of the rotation.

Scheduling: Minimum of 1 resident and maximum 2 residents per block in any combination of PGY1 core residents and senior residents doing an elective. There will be no more than one senior elective resident.

The Division of Child and Adolescent Psychiatry at UWO has a number of excellent opportunities for electives. London has a significant number of children’s mental health facilities which offer a wide range of expertise in the spectrum of child and adolescent psychiatry problems. The various agencies include: CPRI, Madam Vanier Children’s Service, St. Joseph’s Hospital Regional Mental Health Care London and LHSC-Victoria Hospital.

The Family Medicine resident could choose an elective for many purposes, such as learning to interview a child, adolescent, or family: to learn when to refer to a child and adolescent psychiatrist, to develop an understanding of common child and adolescent psychiatry problems, to develop confidence in using psychotropic medications in children and adolescents, or to learn some psychotherapy skills for the treatment of children and adolescents. The types of problems that a resident may be exposed to include: mood disorders, anxiety disorders, eating disorders, suicidal behaviour, disruptive behaviour disorders such as attention deficit with hyperactivity disorder, oppositional defiant disorder and conduct disorder, learning disorders or developmental disorders such as pervasive developmental disorders.

The Department of Psychiatry offers a wide variety of rotations for Family Practice Residents. Inpatient, outpatient, emergency, and psychotherapy experiences are generally available at any time during the year. Interested residents are encouraged to contact Eva Adams via email to discuss their educational needs and availability. Participation in the psychiatry call schedule is expected for off-service residents at a frequency of about 1/5.

The Division of Geriatric Psychiatry at U.W.O. offers elective opportunities for Family Medicine Residents whereby residents can gain experience conducting assessments of older individuals who present primarily with mental health issues. Residents will have an opportunity to participate in both out-patient and in-patient assessments and care.

The types of problems the resident may be exposed to include mood and anxiety disorders in late life, psychological and behavioural problems associated with dementia in late life, dementia itself, psychotic disorders and delusional disorders in late life and delirium assessment with associated work-up. They will also have an opportunity to work as a member of a multi-disciplinary team and within community settings, such as long-term care facilities and retirement homes, and liaise with community partners such as PIECES-trained nurses, nurse practitioners, mental health providers such as CMHA, Geriatric Mental Health and Addiction Services.

The Division can take one resident per block. Advanced planning is necessary due to the limited number of supervisors available. The residents will also have an opportunity to work in the following settings: St. Joseph’s Health Care London, Regional Mental Health Care London, the Southwestern Ontario Regional Psychogeriatric Program and London Health Sciences Centre’s Geriatric Mental Health Program. The Education Co-ordinator – Dr. Lisa Van Bussel –contact at least six months prior to intended start date (as there are limited supervision opportunities) to clarify goals and expectations and to ensure that the Resident will be able to connect with his or her Supervisor.

Objectives: The Family Practice resident will learn methods of interviewing and formulating cases for purposes of diagnosis and treatment. The resident will learn methods of pharmaco and psychotherapeutic intervention for out-patients with acute and urgent psychiatric disorder.

Activities: The rotation will provide experience in working in a clinic which focuses on providing interventions to reduce and stabilize symptoms of acute psychiatric illness. Emphasis is placed on psychological, social and biological approaches to treatment. Residents will work under the supervision of a psychiatrist, attending rounds, observing interviews, diagnostic assessment and treatment sessions (where possible). Students are expected to assume responsibility for evaluation of patients as assigned by the supervisor. There may also be an opportunity to attend Department of Psychiatry CME events, and to work with other individuals and disciplines within the clinic.

A videotape library of demonstration psychotherapy interviews is also available to interested trainees, and learning in CBT (cognitive behaviour therapy) can also be arranged during the rotation.
