The St. Joseph’s Primary Care Diabetes Support Program (SJHC PCDSP) is a provincially funded, outpatient diabetes management program that delivers patient care in an innovative model. This model incorporates multidisciplinary allied health professionals (NP, social work, RN, RD – CDE) together with physician support to provide care for more than 3000 patients in London and surrounding area. Complementing diabetes care, the program also has specialized expertise in treating diabetes-related foot ulcers and wounds.
During this 4-week elective, the resident will gain an approach to providing both pharmacological and non-pharmacological (exercise/nutrition) treatment to patients with both type 1 and type 2 diabetes while working with the multidisciplinary healthcare team.
PCDSP particularly emphasizes the care of patients that may have complex multi-morbidity, be orphaned, and/or potentially have other social determinant of health challenges including but not limited to; language barrier, mental health, poverty, etc.
Call: no call responsibilities
Objectives Include:
- Developing comfort using various diabetes medications including insulin
- Assessment and basic management of diabetes-related foot wounds. This may include familiarity with identifying high-risk feet, peripheral vascular disease assessment, antibiotics, dressings and debridement
- Increased competence of managing vascular co-morbidities including cardiac and renal disease, hypertension and dyslipidemia
- Comfort in case-managing medically and socially complex patients
- Collaborating effectively within an interdisciplinary team model
Scheduling: Can accommodate 1 resident for a 4 week block. Max: 12 residents/year.
