Verification of Undergraduate Medical Education: Request Form
If you require a copy of your transcript, you may order it from the Registrar's Office and request that it be mailed directly to the address you wish to send it. Do not request for it to be sent to UME or we will not receive it.
If you require a copy of your diploma, you may order it from the Registrar's Office and request that it be mailed directly to the address you wish to send it. If you require a certified copy, you may have it sent to yourself, then upload a scanned copy through the form below. We can certify it and forward it electronically as directed, but we are currently unable to mail a hard copy of the document.
