Fill out the appropriate applicaton forms(s):

  • Use the "Mainpro+ Application Form" to obtain credits for family physicians;
  • Use the "MOC (Section1) Application Form" to obtain group learning activity credits for specialists;
  • Both application forms should be submitted for courses that will be attended by both family physicians and specialists.
  • For MOC (Section 3) activities, please note that there are different application forms and standards for Knowledge Assessments and Simulation Activities.

Send the application form(s) and all required supporting documents electronically to CPD.  Regular processing time is 2 - 4 weeks after receiving a completed application, which includes all the attachments and signatures.
