Verification of Postgraduate Training
The Postgraduate Medical Education office provides official verification of training for former residents and fellows. This documentation confirms your dates of training and is typically required for medical licensure or employment.

Submit a Request
Request source verification of past residency or fellowship training.
- Residents and fellows who have completed their training
- Individuals applying for medical licensure or employment
- Third-party credentialing agencies (e.g. FCVS)
- International or provincial medical boards verifying past training
All requests must include:
- A written request confirming the name of the resident or fellow, training dates, and program
- A signed consent form
- Payment (see fees below)
- Requests are typically processed within 3 weeks
- Delays may occur if:
- Required documentation is missing
- Program Director verification is needed
- Standard verification letter – $50 CAD
- Verification of performance and training – $75 CAD
- Online payment only
- Accepted: Visa or MasterCard
