Certificate of Training Completion

The Certificate of Training Completion is the official document that confirms successful completion of a residency or clinical fellowship program. The certificate includes:

  • Western University and Schulich School of Medicine & Dentistry name, crest, and seal
  • Signatures of the Program Director/Fellowship Supervisor and Associate Dean, PGME
  • Resident or Fellow name, as requested
  • Medical degree designation
  • Program name
  • Training start and end date
A separate certificate is issued for each completed program, multiple programs are not combined on a single certificate.

Eligibility

The PGME Office will issue a certificate to registered trainees who have:

  • A request form submitted by their residency Program Administrator or Program Director, or Fellowship supervisor or administrator
  • Completed registration for their final academic year
  • Successfully completed their residency training program in accordance with the requirements of the Royal College of Physicians and Surgeons of Canada (RCPSC) or the College of Family Physicians of Canada (CFPC)
  • Successfully completed the goals and objectives of their clinical fellowship program

Certificates are not issued for:

  • Elective training
  • Incomplete or unsuccessful training

Certificate Guidelines

The name of the trainee will appear on the certificate as it appears on the certificate request form submitted by the training program. It is the responsibility of the trainee to ensure that the appearance of their name in the request form is correct prior to the issuance of the certificate.
The name of the fellowship as it appears on completion of training certificate will reflect what is submitted on the certificate of training request form submitted by the program. If the fellowship program name conflicts with what has been submitted to PGME in the original fellowship appointment, PGME will clarify with the fellowship program.
The designation used on the certificate is Name, Degree without duplicating the Dr. honorific. e.g. John Smith, MD not Dr. John Smith, MD. The degree designation used on the certificate must match the degree conferred by your medical school of graduation as listed in the World Directory of Medical Schools, e.g. MD, MBBS, MB BCh BAO.
International governments and organizations sometimes require documents be authenticated before they will accept them. The PGME Office will respond to third-party requests for verification of the issuance of the  certificate.

The verification function relies on the fact that, under the Ontario Freedom of Information and Protection of Privacy Act, confirmation of a degree, diploma or certificate is public information. This is the only information that the PGME Office can release without the trainee’s explicit consent.

Other information, including confirmation of enrollment, can be released to a third party only with the explicit consent of the trainee. For more information, please visit our Verification of Training page.

After your program submits the certificate request form to PGME, the original paper document is printed and sent to the contact person indicated on the request form, in order to obtain your Program Director or fellowship supervisor's signature. After the hard copy document is signed, your program is responsible for providing you with your original certificate document. PGME does not keep signed copies of your certificate.

If the original certificate has been lost or destroyed: contact the PGME office for instructions to obtain a replacement certificate.

The certificate request form must be submitted to PGME by the Program Director, Administrator or Fellowship Supervisor. Residents and fellows cannot submit their own certificate request.
