To be considered, please complete the screening form below.
What’s next?
- After submission, a staff member will reach out to book your screening appointment.
- Wait times can vary depending on student availability and the number of applicants.
Your first visit is a short assessment to determine your dental needs and whether our student clinicians can provide your care. If you're a good fit, you’ll be matched with a student who will follow up to schedule a full examination. Please note: wait times can vary depending on student availability and academic schedules.
For treatment to progress smoothly you should:
- Be available for at least one clinic session every week. Your student will plan an appointment schedule for you.
- Each appointment may be up to three hours in length.
- Please be on time.
- If you cannot keep an appointment, please contact your student at least forty-eight (48) hours before the appointment. This will allow the student to appoint another patient.
- Cancellation of two booked appointments with less than 48 hours notice will result in dismissal from the clinic.
- Pay regularly as treatments are completed.
Patients and prospective patients are advised that not everyone is suitable to become or remain a patient in the teaching clinics of the Schulich School of Medicine & Dentistry Dental Clinics due to the teaching requirements for our students. The following criteria may indicate unsuitability:
- A lack of medical information
- Extensive and/or complex dental treatment required
- Patient management challenges are beyond the capability of an undergraduate student
- Complex medical history
- Dental treatment needs which are not compatible with our educational program
- A combination of any of the above criteria
