The AAPAN Guide & JRAF Guide have been updated to include the requirement for a letter from the Chair and signed by the faculty member confirming the terms of the appointment/stipend. In addition, template letters have been posted on the Faculty Forms page to cover the above requirements.
UPDATE: July 19, 2012: The template letters have now been moved to the intranet. Please log in using your SSO credentials via the login link at the top right of the screen. They are posted under the Administrators menu under Internal Faculty Documentation.
