Resident & Clerk Cell Phone User Agreement

In submitting this form I confirm I will be receiving or have received a cell phone from the Schulich School of Medicine & Dentistry - Windsor Campus for my professional use during the duration of my stay in Windsor. Cell phones are for the exclusive use of Windsor trainee's.

I agree to return the cell phone in full working order to the Schulich-Windsor Campus upon completion of my training.

Postgraduate trainees acknowledge and agree that a fee of $125.00 will be deducted from my pay should I fail to return this cell phone or if this cell phone is found to be in disrepair beyond its normal wear and tear.

Undergraduate students acknowledge and agree that a fee of $125.00 will be owed to the Schulich - Windsor Campus if they fail to return a cell phone, or the cell phone is found to be in disrepair beyond normal wear and tear. Failure to pay the fine will result in notification on the student's record at Western University.

Any long distance calls or web search charges will be charged directly to you the user of this cell phone.
