The Sandra Banner Student Award for Leadership

Purpose
The purpose of this annual award is to encourage the development of future leaders in medicine, through public recognition and financial support of undergraduate or postgraduate medical trainees who have demonstrated interest in/aptitude for leadership.

Applicability
This award is accompanied by up to $5,000 to support learners' proposed leadership activities, e.g. Leadership conferences (including registration, travel and accommodation), formal leadership education at an accredited educational institution, or for a leadership elective (with defined leadership development goals).

Presentation
The award will normally be presented each year at the annual CaRMS Forum, or at another suitable time/location as determined by the Awards Committee.

Criteria and Process

To be eligible for the award, applicants must be undergraduate medical students or postgraduate medical residents/trainees enrolled full-time in a Canadian medical education program, and residing in Canada. Winners will be selected based on the strength of their individual submissions and accompanying letters of support. Proposed leadership activities will be considered in the context of supporting the applicant's submission.

Application Process
Submissions for the 2014 Sandra Banner Student Award for Leadership will be accepted starting October 15, 2013. Closing date for receipt of applications and supervisor reference/support letters is December 15, 2013,17:00 ET. Late applications will not be considered.


1. Applicant Submission:
Describe in detail the proposed leadership activity that the funding associated with this award will help to support. (NOTE: Applicant submission must be no longer than 10 pages, including CV).
A) What is the nature of the proposed leadership activity?
B) What is the purpose of the proposed leadership activity, i.e. To help you acquire new leadership skills, or to enhance existing leadership skills?
C) What is the intended community scope/impact of the proposed leadership activity, i.e. Local, regional, national, or international?
D) Provide a proposed budget for this activity (e.g. For a leadership conference include: registration, travel and accommodation costs; for leadership education include: tuition costs).
E) Provide your Curriculum Vitae (CV), ensuring that the following personal contact/identity information is included:

• Last Name; First Name;
• Date of Birth (mm/dd/yy) (for identification purposes only);
• Mailing Address;
• Home Phone; Work Phone; Mobile Phone; and
• Citizenship: Canadian or Permanent Resident.

2. Supervisor Submissions:
Request a maximum of three (3) letters of reference/support from your supervisors, of which at least two (2) letters are from any of the following: Dean of Faculty of Medicine; Undergraduate, Student Affairs or Postgraduate Dean; program director; or manager/director of a professional organization. Letters must be emailed directly from the supervisors to carms.

Letters should address:
I. The letter author's relationship to you the applicant;
Ii. Your activities/qualities demonstrating leadership interest/aptitude to date;
Iii. Your demonstrated outstanding leadership skills during education (degree);
Iv. Your contributions to the advancement of the profession of medicine, and/or the quality of medical education; and
V. Evidence of your strong role modelling and mentorship ability among your peers.
Submissions for the 2014 Sandra Banner Student Award for Leadership will be accepted starting October 15, 2013.

Closing date for receipt of applications and supervisor reference/support letters is December 15,
2013, 17:00 ET. Late applications will not be considered.


3. Sending Application and Supervisor Submissions:

All applicant and supervisor submissions must be sent by email, as MS Word or PDF attachments, to: awardscommittee@carms.ca
With the subject title:

4. Application Adjudication:

Applications will be adjudicated January - February 2014 by the Awards Committee of the Canadian Resident Matching Service (CaRMS). The winner will be announced at the 2014 annual CaRMS Forum, or at another suitable date/location as determined by the Awards Committee.

5. Winner's Report:

Winners will be asked to submit a report to CaRMS within 30 days following completion of the activity for which this funding is
awarded. The report should be 3 - 5 pages long and include 1 - 2 photos related to the funded activity. If a winner does not
complete the activity for which this funding is awarded, the winner will be required to repay the full amount of the award to
CaRMS.
