Dr. McAlister is the Angus D McLachlin professor of surgery at the University of Western Ontario. He is a member of the council of the Royal College of Physicians and Surgeons of Canada and chair of the regional advisory committee for Ontario and Nunavut (RAC 3). He also serves as the co-editor-in-chief of the Canadian Journal of Surgery and the editor of historyofsurgery.ca. Lieutenant Colonel Vivian McAlister is a regular force member of the Royal Canadian Medical Service of the Canadian Armed Forces. Dr McAlister is a director and honorary secretary (Canada) of the James IV Association of Surgeons. He is a fellow of American Surgical Association and a member of Harvey Club of London (Canada), the Surgeons Travel Club and the American Osler Society. Dr McAlister has been president of the Canadian Society of Transplantation, chairman of the Canadian Organ Replacement Registry and chairman of the research committee of the Canadian Association of General Surgeons.

Dr. McAlister is a surgeon at University Hospital London with special interest in hepatobiliary, endobiliary (ERCP), transplant and combat surgery. He was a regular force member of the Royal Canadian Medical Service, Canadian Armed Forces, (rank - Lieutenant Colonel, chief of General Surgery) and has deployed overseas many times.

Dr McAlister was editor-in-chief of the Canadian Journal of Surgery. He was a council member of the Royal College of Physicians and Surgeons of Canada. Dr McAlister was chariman of the Royal College's Regional Advisory Committee (RAC 3) for Ontario and Nunavut. He has served as president of Canadian Association of Transplantation and chairman of the Canadian Organ Replacement Registry

Dr McAlister is editor of the web-archive www.historyofsurgery.ca. He has edited a series of articles commemorating Canadian univsersity involvement in medical care during the First World War

Dr McAlister conducts research into his areas of clinical interest including their basic science and history. He has published over 200 papers in areas including the following: transplantationcombat surgery; and history of medicine and surgery. Current research projects.

In 2014, Dr McAlister was awarded the Angus D McLachlin Chair in Surgery. He was awarded the 2019 John McCrae Memorial Medal by the Canadian Medical Association. In 2019, he received the Lifetime Achievement Award of the Canadian Association of Transplantation.
