Learning objectives:

1. Identify potential factors that increase the likelihood of facilitating a home death.
2. Explore recent literature in respect to location of care and service utilization at the end of life.
3. Review local practice data regarding facilitated transfers and congruence of preferred and actual location of death.

 

25% interaction will be included in each session.
