Credentialing/Verification of Training
This credentialing is for Residents and Clinical Fellows who have finished their training and are applying for a license to practice medicine. Former medical students who have completed the M.D. program at Western University and require verification of attendance and/or degree, please click CLICK HERE.
CONSENT FOR RELEASE OF INFORMATION
All verification requests for Residents and Clinical Fellows must be accompanied by a signed "Consent for Release of Information" from the physician. Processing of requests may take up to 2 weeks, or longer if all documentation needed is not present.

We provide 2 types of verification letters.
- VERIFICATION FOR TRAINING DATES only -- $50.00
- VERIFICATION OF PERFORMANCE AND TRAINING DATES -- $75.00
All associated fees are listed in CDN dollars. Cheques can be made payable to “Western University”.
