T2202A Forms

The T2202A forms for the 2012 calendar year will be sent out by the end of February to your Western email account. For those trainees who still have access to Single Sign-On you will be able to print off the form there if you do not receive it by the end of February. For those trainees who have since left and do not have access to Single Sign-On, we will be using their Alternate Email Address. Please check your "SPAM/Junk" folder as the email might have appeared there.

There is no monetary amount associated to a T2202A form. These forms only verify the number of months a trainee was in full-time training from January 1, 2012 - December 31, 2012. Regarding the utilization of this form you should consult your accountant or the CCRA website -

As a reminder, you may also receive the following tax forms in February from London Health Sciences Centre:

T4 Slips Reporting Income
Will be mailed to the same address as your pay stubs by LHSC HR by the end of February. If you are not receiving your pay stubs you will need to forward their current address to monica.mckay@lhsc.on.ca as soon as possible and Monica will try to update prior to the slips being produced. If you have not received your T4 by March 20, 2013 please contact jennifer.galant@lhsc.on.ca for a duplicate.

Conditions of Employment forms (T2200)
Allows trainee to deduct employment expenses such as CMPA fees, certain cell phone expenses, auto expenses etc. If you wish to obtain one of these forms, please have them email monica.mckay@lhsc.on.ca. Fellows should be directed to their Departmental AO if they have any questions about this form.
