- Event Information Form - for recording event information for programs to be rated
- Phase One Data Entry Form - for use by your raters to conduct a content review of presentation materials (prior to the live presentation)
- Phase Two Data Entry Form - for use by your raters when auditing a live presentation
- Phase Three Data Entry Form - for use by participants of live events to capture individual evaluation and feedback
- Live Event Scheduling Template - to assist in coordinating selected educational sessions and the raters who will be conducting live audits at these sessions
